Implantes de Glúteos / Glutes Silicone Implants

En pacientes donde la grasa no es suficiente para una lipotransferencia, la conducta a seguir es aumentar los glúteos por medio de implantes de silicón sub musculares (debajo del músculo), con la finalidad de darle una mayor proyección y redondez a esta zona. En algunos pacientes completo la técnica con grasa subcutánea en los cuadrantes inferiores. Los resultados son excelentes. Aquí les comparto un caso de reciente evolución a las dos semanas de post-operatorio.

Lipoinyección Glútea / Lipoinjection on the buttock

Duración de la grasa inyectada en los glúteos / Timespan of the fat injected into the buttocks

Muchas pacientes cuestionan la duración de la grasa inyectada en los glúteos. Les diré que cuando la grasa se prepara bien y se inyecta en el lugar adecuado tiene una duración de años, aún después de bajar de peso.
La grasa es avascular, es decir, no tiene circulación propia y necesita un lugar que le provea la suficiente circulación para crear la red que la va a mantener viable y perdurable. Esa es la clave de una buena y adecuada técnica. La experiencia en la colocación de la misma es fundamental. Diríamos que la grasa es casi permanente mientras existan las condiciones adecuadas; y lo mejor de todo, podemos colocar o inyectar grandes volúmenes que nos garantizan una mejor forma y glúteos más redondos sin el peligro de utilizar sustancias prohibidas e ilegales como los biopolímeros. Además, hacemos una técnica doble, ya que se realiza una lipoescultura y esa es la grasa que utilizamos para aumentar los glúteos.
Presentar fotos de mujeres modelos son muy lindas, pero no es lo que abunda, las que más se operan son las que necesitan éstos resultados, aquí se los comparto.

La analítica básica requerida para una liposucción es:
- Hemograma completo
-Tipificación sanguínea
-Tiempo de protrombina (TP)
- Tiempo de Tromboplastina Parcial (TTP)
- Glicemia
- Úrea
- Creatinina
- VIH
- HVC
- HBSAG
- Orina
- En pacientes diabéticos o pre-diabéticos se solicita una hemoglobina glucosidada y pruebas hepáticas.
- Radiografía de tórax
- Evaluación cardiovascular.

The basic analysis required for a liposuction is: 

- Complete Blood Count (CBC) 

- Blood typing

- Prothrombin Time

- Partial Thromboplastin Time 

- Glucose 

- Urea

- Creatinine 

- HIV

- HVC 

- HBSAG 

- Urine 

- In pre-diabetic or diabetic patients glucosidase hemoglobin tests and liver tests are required 

- Chest Xray

- Cardiovascular evaluation.

Anestesia por Bloqueo / Anesthesia by Blocking

Casi todos los procedimientos de cirugía plástica se pueden realizar con anestesia por bloqueo. Existen dos tipos de bloqueo por la espalda: bloqueo epidural y bloqueo subaracnoideo. Mi equipo de anestesia prefiere el bloqueo subaracnoideo por considerarlo menos agresivo, más estable, prácticamente no deja efectos secundarios como cefalea (dolor de cabeza), náuseas y vómitos, ni dolor de espalda post-operatorio. El bloqueo subaracnoideo se realiza con una aguja muy fina, no se deja catéter y se utiliza una mezcla de medicamentos para prolongar la analgesia varias horas después de haber terminado la cirugía, lo que le garantiza al paciente mayor confort.

Anestesia General inhalatoria con Sevorane: hablar sobre anestesia general es abundar, ya que todo el mundo conoce este tipo de anestesia. En manos expertas es extremadamente segura. 

Almost all plastic surgery procedures can be performed with anesthesia by blocking. There are two types of blocks in the back: epidural and subarachnoid spinal block. My anesthesia team prefers subarachnoid spinal block anesthesia because of considering it less aggressive, more stable, virtually with no residual effects like headache, nausea and vomiting or post-operative back pain. The subarachnoid spinal block is performed with a very fine needle, no catheter is left and a mixture of anesthetics is used to prolong analgesia for several hours after finishing the surgery, which ensures comfort to the patient.

General inhalation anesthesia with Sevorane: to talk about general anesthesia is abound, as everyone knows this type of anesthesia. In experienced hands is extremely safe.

Rinoplastía / Rhinoplasty

La nariz es el marco de la cara, una nariz que no armoniza con la cara destaca como el elemento discordante en el rostro, llamando la atención de una manera negativa. Aún con grandes modificaciones, la nariz debe notarse natural, que vaya acorde con el resto de la cara. La dismorfia nasal, que es como se conoce una nariz inestética, se puede modificar quirúrgicamente por medio de anestesia local y sedación ó anestesia general.

The nose is the frame of the face, a nose that does not harmonize, stand out as the discordant element, drawing attention in a negative way. Even with major changes, the nose should be noted natural, to be consistent with the rest of the face. Nasal dysmorphia, which is like an unsightly nose is known, can be modified through surgery with local anesthesia and sedation or general anesthesia.

El intrusismo en Cirugía Plástica / Intrusion in Plastic Surgery

Uno de los más graves problemas que enfrenta nuestra especialidad de cirugía plástica en la actualidad es: EL INTRUSISMO. Esto consiste en ejercer una especialidad sin ser especialista en la materia, muchas veces hasta sin ser médicos. No sólo en la República Dominicana hay innumerables intrusos, médicos charlatanes que incursionan en cirugía plástica con la única intensión de ganar dinero, porque es bien sabido que los procedimientos de cirugía plástica no son cubiertos por los seguros médicos. Diversos especialistas, principalmente ginecólogos, dermatólogos, cirujanos generales y hasta médicos generales y enfermeros se creen en la capacidad de realizar éstos procedimientos. En los EEUU de Norteamérica , como hay pocos especialistas en cirugía plástica en proporción a la población, es uno de los lugares con mayor número de intrusos. Llegan a nosotros un sin número de pacientes mal operados procedentes de ese país. Pero esas son cosas que no se publican. Nosotros tendremos que aprender a escribir y a publicar más, de esa forma los pacientes estarán más protegidos. El saber elegir, el investigar y preguntar debe ser tarea obligatoria de toda persona que piense utilizar estos servicios. No se deje engañar, póngase en manos expertas, certificadas y cualificadas. Si se va a realizar un procedimiento en la República Dominicana escoja entre los miembros de la Sociedad Dominicana de Cirugía Plástica, SODOCIPRE. Si ese médico no pertenece a la Sociedad de su país, tenga cuidado. 

One of the most serious problems that our plastic surgery specialty is facing nowadays is: INTRUSION. This consists in practicing a specialty without being specialized on the subject, many times even without being a doctor. Not only in the Dominican Republic there are countless intruders, charlatans doctors who ventures in plastic surgery with the sole intention of making money, because is well known that plastic surgery procedures are not covered by medical insurance. Various specialists, mainly gynecologists, dermatologists, general surgeons and even general practitioners and nurses believe they are capable of performing these procedures. In the United States of America, as there are few specialists in plastic surgery in proportion to the population, is one of the places with the highest number of intruders. They come to us a number of poorly operated patients from that country. But those are things that are not published. We have to learn to write and publish more, thus patients will be better protected. Knowing how to choose, researching and asking has to be mandatory task of everyone who plan to use these services. Do not be fooled, place yourself under expert, certified and qualified hands. If you are having a procedure in the Dominican Republic choose between members of the Dominican Society of Plastic Surgery, SODOCIPRE. If the physician is not in the Society of his/her country, beware.
